Case Management Services
Based on triage criteria, and proactive triggers, a Certified Case Manager
will identify cases in which quality of care, cost-effectiveness and case outcome can be
positively affected. Type of cases which benefit from case management include,
castastropic injuries and illnesses, and cases which involve high cost, high utilization,
and high risk. A V/G Associates Certified Case Manager provides the following
services:
- Telephone communication with patients, family, and healthcare providers. Our
clients may request on-site services as well.
- Collaboration with our client to develop, coordinate and monitor a goal orientated
treatment plan to maximize use of available resources while reducing fragmentation of the
patient's care.
- Short term intervention to streamline delivery of health services while promoting cost
effective alternatives, timeliness and appropriate utilization of available resources.
- Written documentation of case activity, progress and cost savings.
